Primary Care & Wellness Visit - 40 min

Working alongside your Primary Care Provider (PCP), we are responsible for health maintenance, disease prevention, chronic disease management, and health promotion. We offer support to both you and your Primary Care team to monitor the progress of your care. After an initial visit with your Primary Care team (whether after a procedure, an office visit, health screening, etc.) our team coordinates with your Primary Care team for continuity of care. We are here to complete a nursing assessment of your condition, develop a plan of action that coincides with your established care plan, implement steps to help you carry out the plan, and evaluate the effectiveness of the plan with your Primary Care team. Care Plan & Treatment Plan Review - 30 min

If you find yourself needing assistance reviewing or understanding the plan your Provider has given you, our team is available to thoroughly review the plan with you. We take the time to review in detail your care plan, treatment plan, after office visit summary, hospital discharge summary, and post procedure instructions. Patient Education - 20 min

If you have questions or concerns after speaking with your care team about your health condition, a medication, a referral, or treatment course (whether in an office, hospital, or other healthcare facility setting), our team is available to discuss your concerns and answer your questions. Health Advocacy - 30 min

If you face challenges navigating the healthcare system, communicating with your care team, or making beneficial health decisions, our team is available to offer insight related to these challenges. We provide you with tools, resources, and advocacy skills from a healthcare professional’s experience and viewpoint that you can use to get the best results while you interact with your care team as a healthcare consumer (i.e., patient or family of a patient). Consultation - 60 min

Information, guidance, and advisement is provided from a nurse’s healthcare knowledge to address a specific health related topic of your choosing. An in-depth discussion takes place, helping to bring about further understanding that points you in the right direction. Wellness Coaching - 60 min

If you are choosing to take charge of your health and take a proactive approach to your overall wellbeing, our team empowers you and offers exclusive guidance as you work to embrace healthy behaviors and make changes to reach your wellness goals. We support you in making better choices, help you set and achieve your goals, and assist you to live a healthier lifestyle. Considering the whole person (emotional wellness, social wellness, physical health, nutritional health, mental wellness, etc.), we focus on protecting your health, promoting a well lifestyle, and maintaining your overall health and wellbeing. How much does Telehealth Nursing & Wellness Care services cost?

We offer a Fee for Service pricing structure. This means we will request payment (fee) from you for the service(s) we provide. Although our pricing structure is Fee for Service, we take pride in offering value-based services to ensure high-quality care and a valuable telehealth experience.

The cost for each service is listed below:

Preventive Care & Wellness Visit  –  $100

Care Plan & Treatment Plan Review  –  $70

Patient Education  –  $50

Health Advocacy  –  $90

Consultation  –  $120

Wellness Coaching  –  $150

*At this time all services are self-pay: The fee for the specific service you choose will be paid while scheduling your telehealth appointment. After your appointment, you have the option to file your own claim with your insurance company for the service you received. If you choose to file your own claim, be sure to check with your insurance company prior to scheduling your appointment. (*Please Note: we are currently in the process of accepting additional insurance)

How do I know my Protected Health Information (PHI) will be protected and secure?

The video communication system’s database that we use is encrypted allowing full patient data protection. It utilizes intrusion detection systems to continuously monitor its systems for potential threats that may occur at any time. This allows the confidentiality of our patient’s identification to remain protected. The video communication system is fully Health Information Portability Accountability Act (HIPAA) compliant, as well as General Data Protection Regulation (GDPR) compliant. It meets the requirements for properly handling personal data as defined in the law.
